DELAYED EJACULATION

Impaired or delayed ejaculation is actually the increased time period in attaining the sexual climax and releasing the semen (ejaculation) it can also end up with no ejaculation at all. Many medical conditions, medications, stress can lead to delayed ejaculation. However, it is normal if it occurs from time to time.

Symptoms

Needing 30 or more minutes to reach sexual climax, have an orgasm an ejaculate. Or there might be anejaculation (not able to ejaculate)

According to symptoms; delayed ejaculation can be divided into following:

  • Acquired vs. lifelong: acquired is when a person has delayed ejaculation after having a normal period or sexual activity where as lifelong is present from puberty.
  • Situational vs. generalized: situational delayed ejaculation occurs under certain conditions only whereas generalized occurs all the time.
Causes

Psychological:

  • Anxiety, depression etc.
  • Cultural taboos
  • Not good body image
  • Relationship gap or stress causing relationship problems.
  • Performance anxiety

Medications related:

  • Medicines for maintaining blood pressures.
  • Antidepressants
  • Some diuretics
  • Certain Anti-psychotic or anti- seizure medicines
  • Having too much alcohol

Physical causes:

  • Prostate surgeries
  • Birth defects
  • Nerve injury; specifically those that govern orgasm
  • Hypothyroidism or hypogonadism
  • Semen going backward into bladder (retrograde ejaculation)
  • Infections
  • Spinal cord related injury; neurological diseases.
Diagnosis

Diagnosis is made through examination and proper history. There could be an underlying problem that is causing it for which tests are done

  • Blood tests: to look for any cardiovascular disorder, decreased testosterone, diabetes, and other health problems.
  • Urinalysis: for infections or other underlying condition.
  • Physical exam: thorough examination of testicles and penis to look for any abnormality
Treatment

Treatment includes: medications for underlying cause, counselling psychologically(psychotherapy), decreased alcohol or illegal drug use.

  • A Parkinson’s medication (Amantadine)
  • An allergy medication (Cyproheptadine)
  • An anti-anxiety medication (Buspirone)
